The undersigned client affirms that he/she is in good mental and physical health condition for participating in activities offered by Nicole Hanold and/or staff at Soma Studio OC.
Client shall not use alcohol or any recreational drugs while participating in any activities offered by Nicole Hanold and/or staff at Soma Studio OC.
The Undersigned client agrees and affirms that he/she will be participating in activities offered by Nicole Hanold and/or staff at Soma Studio OC at his/her own risk and that he/she is aware of the inherent risks in participating in Somatics, breath work, Dance, and other mind/bodies therapies.
Client agrees to participate in sessions at his/her pace. Client fully releases and holds harmless Nicole Hanold at Soma Studio OC and its teachers, directors, administrators and other clients for any injury sustained during or arising from participation in any activity or program offered by Nicole Hanold at Soma Studio OC or from any responsibility for the functioning of any of the equipment used, owned, rented or borrowed by the client.
Client is aware of current rates and fees, agrees to pay all monies in full on a pre-pay basis .
Client agrees to be photographed or recorded by Nicole Hanold and/or staff at Soma Studio OC to use only on the company website or any social media outlets such as (YouTube, Facebook, Instagram, Twitter and etc... The liability release form embodies and contains the entire agreement and understandings for the parties.
The undersigned clients has read, understands, acknowledges and agrees to the above described terms and conditions of their agreement with are binding on the client and his/her heirs, legal representatives and successors. This agreement is entered into in the state of California, and should be construed in accordance with and governed by the laws of the state of California. Instructors and schedule are subject to change at any time without notice. Nicole Hanold and/or staff at Soma Studio OC reserves the right to refuse service.
